We are a team of healthcare professionals – nurses, dieticians, mental health practitioners, pharmacists, exercise physiologists, and psychiatrists – who are dedicated to supporting your health and wellness needs. We deliver a variety of programs and classes to complement the care you receive from your doctor and our PCN team. 

In response to the COVID-19 pandemic, we are offering classes virtually through Zoom video.

Healthy Living

Eating the Mediterranean Way

Learn the health benefits of this well-studied way of eating and how to apply it to your lifestyle.

Eating in the NOW

A unique program designed to free yourself from dieting and heal your relationship with food and your body. You will learn the difference between emotional and physical hunger cues, gain a sense of body wisdom, and make peace with food while developing mindful eating techniques.Each offering is a six-week series that is held weekly for two hours.

  • January 12 - February 16, 2021 - every Tuesday from 2:00 - 4:00 p.m.

  • January 13 - February 17, 2021 - every Wednesday from 5:30 - 7:30 p.m.

  • March 9 - April 13, 2021 - every Tuesday from 2:00 - 4:00 p.m.

  • March 10 - April 14, 2021 - every Wednesday from 5:30 - 7:30 p.m.

Let’s Talk Menopause!

Dealing with hot flashes, night sweats, trouble sleeping, and mood swings related to menopause? Discover ways to help manage these symptoms through lifestyle changes, non-prescription and prescription therapy options, including a detailed discussion on hormone therapy.

Learn about the symptoms and risks related to arthritis. Find out about essential practices to manage chronic joint symptoms and pain.

Virtual Drop-In Exercise Class

Led by an Exercise Specialist, this virtual class focuses on guiding you through low impact stretches and exercises. Each class will focus on a specific muscle group, so you optimize your workout and learn exercises associated with different muscle groups. We do not require medical screenings for participation in this program, so if you have any concerns please contact one of our Exercise Specialists to discuss them.

  • Every Monday and Thursday afternoon, 1:30 - 2:00 p.m.

  • January 4 - April 8, 2021

Supervised Exercise Program

This is a virtual group exercise program designed to help you improve your muscular, cardiovascular, and overall health. This program is suitable for most individuals, including those with stable, chronic conditions.

  • Every Tuesday and Thursday afternoon; 2:00 - 3:00 p.m., January 19 - February 25; or March 2 - April 8, 2021.

Stand Tall to Osteoporosis

Join us for a relaxing discussion about osteoporosis, where you will learn about commonly used tests and treatment options, as well as gain understanding about your risk factors and how to take steps to improve the health of your bones.

Chronic Disease Management

CHANGE program (physician referral or self-referral)

The Canadian Health Advanced by Nutrition and Graded Exercise (CHANGE) program is a ten-week series and focuses on conditions that often progress to high blood pressure, heart disease, stroke, and diabetes. Ideally, participants in this program are ready to engage in lifestyle changes by attending sessions that focus on physical activity, healthy eating, and goal-setting.

  • Option A: January 19 - March 23, 2021; 5:00 - 8:00 p.m. (every Tuesday evening)
  • Option B: January 20 - March 24, 2021; 1:00 - 4:00 p.m. (every Wednesday afternoon)
  • Option C: April 13 - June 15, 2021; 5:00 - 8:00 p.m. (every Tuesday evening)
  • Option D: April 14 - June 16, 2021; 1:00 - 4:00 p.m. (every Wednesday afternoon)

Diabetes Management Program (physician referral required)

This program supports patients in managing blood sugars, blood pressure, cholesterol, healthy eating, medications, insulin, and reviewing test targets. Our healthcare team members provide education and one-on-one support in-person, over the phone, and virtually.

Diabetes 101 – The Basics

Join this virtual class to learn about diabetes, how it is treated, and the potential problems it can cause for your health. Find out how to make changes to your daily life to help you control diabetes, such as managing your blood sugar level, taking care of your feet, and setting personal goals. It is beneficial to take the Diabetes 201 class to learn more about managing your food and lifestyle with diabetes.

Diabetes 201 – Managing Food and Lifestyle with Diabetes

Learn how different foods change your blood sugar and find out how to make snacks and meals so you can better control your blood sugar. We will talk about the importance of physical activity and how to exercise safely. It is beneficial to take the Diabetes 101 class to learn more about living with diabetes.

  • Wednesday, February 24, from 6:00 - 7:30 p.m.

Good Life with Osteoarthritis (no referral required) 

The Denmark GLA:D™ Program – Led by Exercise Physiologists, GLA:D™ is a seven-week exercise-based program for people living with osteoarthritis, which aims to reduce pain, build strength, and improve physical activity.

  • Option A: January 5 - February 18, 2021; every Tuesday and Thursday afternoon; 12:15 - 1:15 p.m.

  • Option B: January 27 - March 12, 2021; every Wednesday and Friday morning; 11:00 a.m. - 12:00 p.m

  • Option C: March 9 - April 22, 2021; every Tuesday and Thursday afternoon; 12:15 - 1:15 p.m.

  • Option D: March 24 - May 7, 2021; every Wednesday and Friday morning; 11:00 a.m. - 12:00 p.m.


This virtual group support program provides you with the tools and skills you need to quit using tobacco. This program runs for six weeks and sessions are 90 minutes.

  • Every Friday for six weeks, February 19 - March 26, from 1:30 - 3:00 p.m.

Mental Health

To access our mental health programs, you require a physician referral.

Mental Health Program (physician referral required)

Depending on your needs, we offer free in-house classes, connections to community supports, and/or sessions with Mental Health clinicians or psychiatrists. 

Programming is available to children (pre-school and up), adolescents, adults, and seniors. Social work services are also available. 

Changeways – Depression Therapy Group (physician referral and Mental Health clinician screening required)

This is a nine-week, evidence-based depression therapy group that provides relevant information about managing depression. We provide a manual to encourage the use of strategies on an ongoing basis.

Acceptance and Commitment Therapy (ACT) – Anxiety Therapy Group (physician referral and Mental Health clinician screening required)

ACT is a nine-week, evidence-based anxiety management group that encourages contact with uncomfortable internal experiences, instead of avoidance of them.

Hold Me Tight Program: A Relationship Education Group for Couples

Learn about essential conversations for a successful relationship, which includes participating in private exercises with your partner and watching video demonstrations. Discover tools that will help you to better understand your partner and improve your relationship.
